  • SPECTATORS: Please have your coach/manager fill out the form.
  • PLEASE wait 24 hrs after your game before filling out and submitting a report
  • BE SPECIFIC: Specify what rules were broken and eliminate generalizations ex. “he was horrible”
  • BE HELPFUL: This form is not a place to vent, it is to be used as a productive tool to improve the league experience
