Submission Guidelines
  • TEAM MANAGERS ONLY: Please have your coach/manager fill out the form.
  • PLEASE wait 24 hours after your game before filling out and submitting a report.
  • BE SPECIFIC: Specify what rules were broken and eliminate generalizations. (ex. “he was horrible”)
  • BE HELPFUL: This form is not a place to vent, it is to be used as a productive tool to improve the league experience.

Thank you for taking the time to complete the on-line referee evaluation form.  We use your comments for training and tracking purposes.  If you would like the Director of Officials person to contact you please provide your name, e-mail and phone number.

Keep in mind that referees are human, and you are not going to always get someone’s best performance every game. On the other hand, we do expect our referees to act in a professional manner, show effort, and be knowledgeable of the rules of the game and of NWISR policies.

RESPECT YOUR REFS!

At Salmon Creek Indoor, our goal is to promote a fun, soccer-loving community. Confronting and verbally abusing referees can often lead to cards, which is no fun for anyone. It’s not realistic to expect a referee to call a perfect match – just like it’s not realistic to expect that players will play a perfect match. If you have a question regarding a referee’s decision, ask them to explain it in a polite, non-confrontational way during half-time or after the match.
All referees at Salmon Creek Indoor are not employees of the facility, but are supplied by Northwest Indoor Soccer Referees (NWISR).
